Applications:
Work experience must be approved before commencement to ensure workplace insurance for employer and student are in place.
- Students completing work experience under the Year 10 Work Experience Program need to submit their application form for approval.
- Students completing work experience outside of Term 2 need to ensure the application is submitted 4 weeks before starting

Students need to print and complete the Agreement Form

The Agreement Form must be completed and signed by the student, parent/guardian, and employer.

Once all documents are signed, they must be scanned or photographed and uploaded to the online application, along with the required details.
Approval Process
- Faith CSDE Future Pathways team will ring the employer during the approval phase.
- Faith CSDE Future Pathways team will review application in line with that industry and proposed activities.
Incomplete applications will not be accepted.

Students, parents and employers will receive approval or non – approval via email.
Prior to starting work

Students on or before the first day of work need to upload the following photos:

Emergency Evacuation Plan / Muster plan

First aid station

Photo of student in the workplace with appropriate PPE if required

Employer may be contacted by staff during the work experience period

Upon completion of work experience an online form will be completed by both the student and the employer
