Faith provides student identification cards for students upon request. Please allow for a 48 hour turnaround time upon filling out the below form.

Photo Guidelines

Please upload a clear image file showing both your head and shoulders on a plain background (e.g passport photo).

Additional guidelines:

  • Take photo against a white or light-coloured plain background
  • Position face to be looking directly at the camera and not tilted in any direction 
  • Position hair off the face so that the edges of the face are visible. Please no hats or sunglasses.
  • Feel free to smile!

Please make sure your address is correct on Sentral (our main database) through the Sentral Parent Portal.

Once the photo is approved by Admin, the process takes approximately 48 hours before it will be ready for collection from Graham St or posted to your address on file.
